Understanding Your Service Agreement with Unique Care Group

At Unique Care Group, we want to make sure you know all about your service agreement. This guide helps explain what a service agreement is and how it works in a simple way.

What is a Service Agreement?

A service agreement is a plan between you and us. It talks about the support and services you’ll receive from Unique Care Group. At the end of this guide, we list all the supports you can get and how much they cost, which is called your Schedule of Supports.

Our Responsibilities to Each Other

Responsibilities are tasks or things we agree to do. There are things you need to do, things we need to do, and some things we have to work on together.

Your Part

  • Let us know how you want your supports delivered.
  • If you need to cancel an appointment, please tell us 48 hours beforehand.
  • Inform us right away if there’s any change to your plan or if it ends.

Our Part

  • We’ll provide you with the supports that meet your needs, when you need them.
  • You’ll get an invoice for your supports at least once every two weeks.
  • We follow all NDIS laws and regulations.
  • We’ll keep all the important paperwork.
  • If we need to end this agreement, we’ll give you 30 days’ notice.
  • We’re here to listen to your feedback and solve any problems quickly.

Working Together

  • Treating each other with kindness and respect is important.
  • We’ll make a plan for your supports together.
  • We’ll check on how your supports are going at least once a year.
  • Let’s keep talking to each other about your supports and funding.

Paying for Your Supports

There are a few ways your funding can be managed:

  • NDIS Managed: The NDIA will pay us directly for your supports.
  • Plan Managed: If you have a Plan Management Provider, they will pay us.
  • Self Managed: You’ll manage your own funding and pay us directly. Don’t forget about travel costs if you’re managing your own transport funding.

We’ll send you invoices, and you’ll need to pay them within 14 days to keep receiving support.

Changing or Ending This Agreement

Sometimes, we might need to change or end this agreement. We promise to talk to you about any changes. If you or we decide to end the agreement, we need to tell each other 30 days before.

Cancelling Services and Giving Feedback

If you need to cancel, please give us 48 hours’ notice. Your thoughts on our service are very important, so feel free to give feedback or make a complaint if something’s not right.
