Understanding Incident Management at Unique Care Group

At Unique Care Group, your safety and happiness are our top priorities. This guide is designed to help you understand what to do if something goes wrong, and how we will support you through it.

What is an Incident?

An incident is any event that shouldn’t happen, including:

  • Harm or potential harm to you or others
  • Damage or loss of property
  • Situations where someone is hurt or could have been hurt
  • Serious issues like abuse, neglect, or if someone is seriously injured

Types of Incidents

Incidents can vary widely, but they include:

  • Experiencing or witnessing harm or abuse
  • Property damage or loss
  • Medical errors, like taking the wrong medication
  • Changes that could harm you, like unexpected changes in support
  • Illegal activities

How to Report an Incident

If you or someone else is involved in an incident, it’s important to let us know right away. You can:

  • Talk to a staff member
  • Send us an email
  • Give us a call

Our Response to Incidents

We take every report seriously and aim to respond as quickly as possible. For less serious incidents, our team will work directly with everyone involved to find a solution. For more serious or “reportable incidents,” we must also notify authorities like the Police and the NDIS Quality and Safeguarding Commission.

Information We Need

To help you effectively, we need details about the incident, such as:

  • What happened and how
  • When and where the incident occurred
  • Who witnessed it
  • When you reported it

How We Support You

Your well-being is our concern. We will:

  • Ensure your safety
  • Support your health and recovery
  • Work with you and your advocate to address the incident
  • Implement changes to prevent future incidents, which may include updating our policies or training our staff

Feedback and Improvement

We welcome your feedback on how we handle incidents. If you’re not satisfied with our response, we encourage you to contact the NDIS Quality and Safeguards Commission. We’re here to assist with this process too.
